Emerging Focus on Age Friendly Health Systems:  A Step Forward in Fall Prevention

As hospitals and health systems prepare to care for our aging population, many are using the Age-Friendly Health Systems initiative to provide care focused around the 4Ms- What Matters Most, Medication, Mentation, and Mobility.  Learn how these are being implemented in NH and beyond, and can increase patient safety and outcomes, increase patient and family engagement, reduce harm from falls, and decrease hospitalizations. We will learn practical and innovative strategies for implementation. While there is a special focus on falls prevention and mobility, we will address how all of the 4Ms interconnect.

Learning Outcome(s)

At the conclusion of this learning activity, participants will be able to discuss the process of implementing specific quality improvement projects to become an Age Friendly Health System including the benefits to both patients and the system itself.
