Total Population Health

The Total Population Health Initiave aims to improve the health and well-being of NH residents by fostering connections between healthcare, public health, and the social determinants of health while advancing health equity.

Our Story:

The Total Populations Health Initiative began in June 2017 following the creation of the Total Population Helath Framework by the FHC Board of Directors and staff. Since then, we have worked to promote the use of this framework among our partners to encourage collaboration and collective action to improve the health and wellbeing of residents while ensuring all communities have equal opportunities for good health.

Our purpose is to foster collaboration, build partnerships, and identify opportunitite sto leverage skills, resouces and expertise to create systems and environments that support and contribute toe the health and well-being of all NH residetns. Our goals include:

  • Build place based population health awareness and knowledge.
  • Create bridges between healthcare, public health, and community organizations and partnerships.
  • Provide a platform to facilitate dialogue and networking among our partenrs to share expertise, identify collective opportunities and catalyze NH based total population health initiatives.
  • Promote the use of evidenced based and promising practices to advance total population health.
  • Identify and coordinate opportunties to access, share and utilize data.
