The Rural Health Transformation Program (RHTP) is a first-of-its-kind, CMS-funded initiative to fundamentally strengthen rural health care across New Hampshire, expanding access to primary care, behavioral health, workforce development, and technology infrastructure in communities that need it most. The Project Director will own the end-to-end delivery of one or more assigned RHTP initiatives.

Grant Compliance Coordinator

The Rural Health Transformation Program (RHTP) is a multi-year, CMS-funded program designed to expand access to primary care, behavioral health and workforce development across rural New Hampshire communities. This role is central to the program’s success, ensuring every dollar is spent compliantly, every deadline is met and every subrecipient is set up to succeed
The Grant Compliance Coordinator is responsible for ensuring effective grant monitoring, federal compliance adherence and comprehensive documentation management for the Rural Health Transformation Program (RHTP).
