The Foundation for Healthy Communities (FHC) is a non-profit organization that collaborates with hospitals, community partners, state departments and insurers on statewide initiatives that improve the health and health care for all New Hampshire residents and their families.

The New Hampshire Hospital Association and the Foundation for Healthy Communities are committed to advancing the health and well-being of all people in New Hampshire. To advance our missions, we value inclusive care that recognizes and affirms every individual’s unique characteristics, culture, and experiences. We believe that all people deserve respect and to be accepted as they are, free from discrimination and bias.

Founded in 1968 as an education and research organization for the New Hampshire Hospital Association (NHHA), the Foundation was reorganized in 1995 to meet the needs of a changing healthcare system. The Foundation for Healthy Communities is led by a board of trustees with a broad range of expertise which guides its strategic and programmatic efforts.   Today, the Foundation is a partnership between New Hampshire hospitals, health plans, home care agencies, community organization and other healthcare stakeholders focusing on quality of care, access to health services, and prevention as the major priorities of work, aligning with the Foundation’s mission of improving total population health for all communities.

Since its inception, the Foundation for Healthy Communities has worked tirelessly to create partnerships to improve health and healthcare in the state.  The success of our statewide efforts in working with diverse partners has led to even more comprehensive collaborations with local communities to facilitate health improvement.

In its work with partners statewide, the Foundation’s key objectives are to improve total population health by promoting innovative, high quality practices

within organizations and communities; to lead change strategies that educate, create, and sustain healthier communities and make the healthy choice the easy choice; and to improve access to affordable health care and resources that support the well-being of all people.

The Foundation has garnered the reputation of being a statewide convener from its fellow peers and partners, and as such, has impacted thousands of people in New Hampshire over the years through the implementation of initiatives and projects that align with its mission and key objectives.   They are renowned on a state and national level for their ability to engage diverse partners in new, innovative collaborations and to serve as the backbone for its various initiatives that address the health and healthcare needs of our state through the collective impact.

The Foundation’s history of success of improving health for all is built upon creating collaborative initiatives with people and organizations throughout the state, many of which started in one or a few communities and then expanded to become statewide initiatives.
