COVID-19 Vaccine Project

The Foundation for Healthy Communities has received a three-year grant the NH Department of Health & Human Services to increase infrastructure and capacity for New Hampshire hospitals to administer and reduce access barriers to COVID-19 vaccinations.

The COVID-19 Vaccination Project will address the critical need to increase access to COVID-19 vaccines and boosters to prevent severe illness and hospitalizations throughout the State. The Foundation will work with New Hampshire’s hospitals to develop and implement engagement strategies to promote COVID-19 vaccinations as part of health care and increase vaccine confidence through education and outreach. 

As part of the grant, the Foundation will oversee the expansion of data system infrastructure with New Hampshire hospitals to expand and implement the Immunization Information Systems (NHIIS), including successful developing pathways for HL7 messaging.

Project Goals

  • Increase accessibility of COVID-19 vaccinations in a variety of inpatient and outpatient settings
  • Provide access to COVID-19 vaccines for underserved populations, populations experiencing disproportionately high rates of COVID-19 and related deaths, and individuals experiencing homelessness or housing instability
  • Providing services to increase COVID-19 vaccine confidence
  • Leverage community effort to increase the knowledge of COVID-9 vaccinations among vulnerable populations
  • Tracking efforts, successes, and challenges in promoting vaccine awareness and uptake of COVID-19 vaccinations, and addressing vaccine misinformation
