Announcing the Advance Care Planning Facilitator Training – Fall 2024

Time of Each Session: noon – 1:15pm
These are interactive sessions and registrants are expected to attend all four sessions (zoom link and pre- session handouts will be sent to attendees each week)
The Foundation for Healthy Communities is offering a four-part series for health care professionals whose work includes facilitating discussions with individuals/families that lead to completion of Advance Directives. The workshop is appropriate for physicians, nurses, Nurse Practitioners, Physicians Assistants, Social Workers, Chaplains, EOL Doulas and other professionals. It will include a review of the statutory requirements for Advance Directives, technical and practical components, and strategies to engage in meaningful conversations.
- Basic concepts of ACP and history of Advance Directives
- Detailed attention to the Advance Directive forms and rules
- Skills in introducing and informing consumers about Advance Care Planning
- Skills in advising health care colleagues about Advance Directives, Medical surrogacy, and guardianship
- An opportunity to discuss challenging cases and conversations
