Falls Prevention

The Foundation for Healthy Communities has a long history with facilitating and participating in programs aimed at reducing the risk of falling with injury among older adults, whether with partner organizations or hospital initiatives. The Foundation partners with the New Hampshire Falls Risk Reduction Task Force, a coalition of professionals from across disciplines who are committed to reducing the risk and numbers of falls among the state’s older adult (65 plus) population.

The goals of the New Hampshire Falls Risk Reduction Task Force are to:

  • Reduce the rate of death and disability in older adults due to falls.
  • Reduce the risk of falling in the older adult population.
  • Educate and train professionals working with older adults on falls.
