Foundation for Healthy Communities Announces Winner of Patient Safety and Quality Improvement Award at 2024 Annual Meeting

CONCORD, NH – At its Annual Meeting recently held October 20 – 22, 2024, the Foundation for Healthy Communities (FHC) bestowed its Patient Safety and Quality Improvement Award on Concord Hospital Health System. The award honors a team of health care professionals who demonstrate a continued commitment to providing quality care to their patients, leading the charge for quality improvement, promoting transparency to improve health care, and achieving better outcomes through the implementation of a strong culture of safety.

 

 

“We were honored to recognize Concord Hospital Health System for going above and beyond to ensure that New Hampshire’s health care delivery system is patient centered and quality focused,” stated Peter Ames, executive director of the Foundation for Healthy Communities.

 

Concord Hospital Health System, Concord, was recognized with the Honoring Excellence in Patient Safety & Quality Improvement Award due to its commitment to delivering the highest quality of care throughout its system. The system focused on improving outcomes due to a strong culture of safety, including launching a skilled and restorative care program to provide a new level of post-acute skilled care within its system; reducing health disparities among specific patient populations; and promoting transparency among care teams, with patients, families and community stakeholders.
