The New Hampshire Medication Bridge Program was facilitated by the Foundation for Healthy Communities to help eligible uninsured and underinsured patients of all ages to receive needed prescription medications. The program worked through trained health care system staff around the state to help individuals determine if they may be eligible for pharmaceutical manufacturer discounts to medications. Although the program is no longer managed by the Foundation for Healthy Communities as of 2021, discounts may still be available to patients by working through the financial services departments of their local health care system.
